<p>一、项目基本情况：<br/>1.项目编号：HN-2023-13<br/>2.项目名称：南阳市骨科医院中医文化标识标牌采购安装项目<br/>3.预算金额：860184.43元<br/>4.采购内容：南阳市骨科医院中医文化标识标牌采购安装项目（详见第五章采购内容）<br/>5.合同履行期限即供货期：签订合同后45日历天内供货、安装、调试完毕；<br/>6.质量要求：合格，满足采购人实际需求<br/>7.供货地点：采购人指定地点<br/>8.质保期：3年<br/>9.本项目不接受联合体。<br/>二、申请人的资格要求：<br/>1.满足《中国政府采购法》第二十二条规定；<br/>2.落实政府采购政策需满足的资格要求：节能产品、环境标志产品、促进中小微企业、监狱企业及残疾人福利性单位发展等政府采购政策。<br/>3.本项目的特定资格要求：<br/>3.1 （1）对列入失信被执行人、重大税收违法失信主体、政府采购严重违法失信行为记录名单及其他不符合《中国政府采购法》第二十二条规定条件的供应商，拒绝其参与本次政府采购活动。投标人需通过“信用中国”网站“列入失信被执行人、重大税收违法失信主体、政府采购严重违法失信行为记录名单”企业和法定代表人的查询，通过中国政府采购网对“政府采购严重违法失信行为信息记录”企业信用记录查询（投标文件中提供网站查询截屏，查询时间公告后有效）。<br/>（2）供应商在国境内注册的，具有有效的营业执照，具有独立承担民事责任的能力，有提供本次采购货物的能力。<br/>3.2本项目不接受联合体投标，不允许分包和转包。<br/>三、获取采购文件<br/>1、文件获取时间：凡有意参加投标者，请于2023年03月15日至2023年 03月21日(法定公休日、法定节假日除外)，每日上午9：00至11：00，下午15：00至17：00（北京时间）；领取招标文件,文件费：500元。<br/>2、方式：现场获取（原件备查，留加盖单位公章的复印件1套）<br/>3.获取招标文件时需提供申请人资格要求证明资料：</p><p><img src="http://www.zhaobiaopai.com/file/upload/202303/14/213427671.jpg" title="" alt=""/></p></div>
